If you have a Minnesota Tax ID Number and a Withholding Tax account, log in to e-Services to file a contractor affidavit electronically.
If you are Minnesota Contractor and do not have a Minnesota ID number or withholding tax account, use our Contractor Affidavit filing system .
If you are an out-of-state contractor who is not registered for Minnesota Withholding Tax, you must complete and mail Form IC134, Contractor Affidavit .
Note: Before the Minnesota Department of Revenue certifies a Contractor Affidavit, we must have all Withholding Tax returns and payments on file for the entire contract period.

If we approve your Contractor Affidavit, you will see a confirmation page. Send this page to the person who hired you.
Note: If you are a single-member corporation, you are considered an employee of your own business. For details, see Withholding Fact Sheet 6 .
To correct errors on a Contractor Affidavit you submitted, complete a new Contractor Affidavit with the correct information. We will compare your original Contractor Affidavit with the new one and note any changes.
Once approved, we will provide you a new affidavit number (or approval letter, if you filed by paper). Provide the new affidavit number or approval letter to the person who hired you.
If you submitted your Contractor Affidavit using e-Services, go to your Withholding Tax account and select the History tab to view previous contractor affidavits.
If you used our Contractor Affidavit system , you should have received an email with instructions on how to view your affidavit. If you no longer have the email, you must submit another affidavit.
